Position Summary

The Registered Nurse (RN) will guide patients and families throughout the healthcare system, providing compassionate, patient‑centered care and advocating for those they serve. The RN will be an essential member of the multidisciplinary team, dedicated to high‑quality nursing services across adult primary care, women’s health, and pediatrics.

Qualifications

  • Graduation from an accredited nursing program.
  • Licensed with the Arizona State Board of Nursing in good standing.
  • Two (2) to five (5) years of clinical experience.
  • Demonstrated application of nursing planning (assessment, diagnosis, goals, implementation, evaluation).
  • Proficiency in clinical record‑keeping systems.
  • Strong written and verbal communication skills.
  • Active Arizona driver’s license and valid automotive insurance.
  • Certification in CPR and AED per American Heart Association or Red Cross guidelines.
  • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety.
  • Compliance with all mandated immunizations and training requirements.

Position Responsibilities

Patient Care

  • Perform triage phone calls and walk‑in patient assessments using approved procedures for adult primary care, women’s health, and pediatric populations.
  • Assist providers with procedures such as wound care, urinary catheterization, suture/staple removal, laparoscopic tissue removal, vaccine administration, and point‑of‑care testing.
  • Document results of EKG, peak flow, spirometry, albuterol, and oxygen treatments in the electronic health record.

Clinical Collaboration

  • Collaborate with the multidisciplinary team to review and act on tasks and provider requests.
  • Support vaccine and medication administration, including inhaled vaccine delivery.
  • Support referral processes per standing orders.
  • Develop patient‑centered self‑management goals (SMGs) in collaboration with patients.

Quality Improvement and Coordination

  • Collaborate to improve Uniform Data System (UDS) quality metrics and Care Delivery Integration.
  • Coordinate outreach to high‑risk patients, including transition of care with health plans, hospitals, and skilled nursing facilities.
  • Identify barriers to care and social determinants of health by ensuring the PRAPARE survey is collected.
  • Support high‑risk prior authorizations and durable medical equipment (DME) as needed.

Leadership and Accountability

  • Provide leadership for collaboration and coordination of services, including delegation of tasks among licensed and unlicensed staff.
  • RNs are fully accountable for all nursing services and associated patient outcomes in all ambulatory care settings.

Patient Education and Reporting

  • Provide education and reporting on STIs and public health initiatives.
  • Build relationships with external healthcare organizations to support patient care.

Administrative Responsibilities

  • Obtain medical records, lab, and diagnostic results as needed.
  • Ensure proper documentation using approved templates in the electronic health record.
  • Conduct medication reviews, reconciliations, and refills.

Skills and Competencies

  • Apply critical thinking and organizational skills to specialty tasks and organizational standards.
  • Enhance patient safety and quality by assessing, setting goals, implementing, and evaluating nursing services per Board of Nursing scope and organizational standards.

Family Medicine / Internal Medicine RN

  • Collaborate on Medicare wellness visits, assessments, and chart documentation.
  • Provide direct nursing care: triage, health assessments, medication administration, vaccinations, record management, and quality improvement.
  • Educate patients and families on health management, disease prevention, and wellness promotion.

Women’s Health RN

  • Perform and interpret non‑stress tests for monitoring fetal well‑being.
  • Assist with pap smears, breast exams, contraceptive education, and pregnancy care.
  • Provide direct nursing care: triage, medication administration, vaccinations, and quality improvement.
