Location: Mesa, AZ (Primary) | Some coverage in Chandler
Setting: Skilled Nursing & Assisted Living Facilities
Schedule: Full-time, weekdays on-site 9 AM – 5 PM, plus weekend rotation once or twice per month
Employment Type: 1099 Independent Contractor
Compensation: Productivity-based pay with potential to earn up to $300,000 annually
About the OpportunityWe’re hiring a full-time Nurse Practitioner or Physician Assistant to provide ongoing care within post-acute and long-term care settings across Mesa and Chandler.
This position offers steady weekday hours, autonomy, and a manageable patient load in a supportive clinical environment.
Providers typically see 15–16 patients per day in Skilled Nursing and Assisted Living communities, focusing on established, stable residents and new admissions as needed.
Key Responsibilities- Provide direct, in-person care at assigned SNF and ALF facilities
- Maintain an on-site presence daily
- Participate in weekend rotation once or twice per month
- Manage 15–16 patient visits per day, including new and follow-up appointments
- Respond to nursing staff calls related to your assigned patients
- Conduct assessments, adjust medications, and manage chronic conditions
- Collaborate with facility staff and interdisciplinary teams
- Document all encounters accurately using Point Click Care (PCC) EMR
- Active, unrestricted Arizona NP or PA license
- Graduate of an accredited U.S.
-based NP or PA program - Preferred: 1–2 years of post-acute, long-term, or primary care experience
- Strong communication, clinical reasoning, and organizational skills
- Reliable, professional, and comfortable working independently
- Consistent weekday hours with limited weekends
- Manageable daily volume (
