As we continue expanding our Population Health programs, we are seeking an experienced and compassionate Nurse Practitioner to join our growing interdisciplinary clinical team.

---# Position Summary This is a unique opportunity for a Nurse Practitioner who enjoys building long-term patient relationships through proactive, technology-enabled care.

This position is centered on providing advanced clinical oversight for patients enrolled in Vanguard Mobile Physicians'
** Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Population Health programs**. Working alongside an experienced team of care coordinators, physicians, home health clinicians, hospice, and palliative care providers, you'll help identify changes in patient condition early, optimize treatment plans, and improve outcomes before hospitalization becomes necessary.  This is not a high-volume urgent care telemedicine position. Our focus is building long-term relationships with Medicare patients through proactive chronic disease management and coordinated interdisciplinary care.

Your primary responsibilities will include:
* Scheduled telehealth follow-up visits with established Medicare patients
* Quarterly comprehensive chronic disease reviews
* Clinical evaluation of patients identified through RPM and CCM programs
* Medication optimization and chronic disease management
* Treatment plan adjustments
* Collaboration with physicians and interdisciplinary care teams

Most patient encounters are scheduled, predictable, and approximately
** 15–30 minutes
** in length.

While this role is primarily focused on telehealth population health services, providers interested in broader clinical opportunities may also have the opportunity to participate in additional Vanguard Mobile Physicians initiatives—including mobile primary care, transitional care, post-discharge follow-up, and other innovative care models—as our programs continue to expand.

* Provide clinical oversight for patients enrolled in Vanguard's Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs
* Conduct scheduled telehealth visits with established Medicare patients
* Perform comprehensive quarterly chronic disease assessments
* Evaluate patients referred by the CCM and RPM care coordination team
* Review remote monitoring data, including blood pressure, blood glucose, pulse oximetry, weight, heart rate, and other physiologic trends
* Assess chronic disease stability and identify changes in condition
* Optimize medications and treatment plans
* Educate patients and caregivers regarding disease management
* Coordinate care with primary care physicians, specialists, home health, hospice, and palliative care teams
* Identify patients requiring in-person evaluation or higher levels of care
* Complete timely, accurate EMR documentation
