Position Summary

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The role is accountable for the quality of nursing services delivered by self or others, utilizing specialized knowledge and judgment to assess data and plan, provide and evaluate care appropriate to the physical and developmental needs of assigned patients.

Functions
  • Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs; reviews patient history and physical with patient/family and assures completion within appropriate time frames.
  • Formulates a plan of care, including the discharge plan, utilizing assessment data and input from patient, family and multidisciplinary team; revises the plan as patient needs change.
  • Implements the plan of care through direct patient care, coordination, delegation and supervision of the health‑care team; promotes continuity of care by communicating patient status accurately.
  • Evaluates the patient’s and family’s response to care and teaching, revises the plan of care as needed, and assesses patient progress toward goals.
  • Documents assessment, planning, implementation and evaluation in the patient record; ensures documentation is legible, timely and policy‑compliant.
  • Provides care based on best evidence; participates in research activities; engages in unit or facility shared leadership; mentors and precepts other nurses as appropriate.
  • Contributes to society through activities that lead to excellent patient outcomes; participates in professional organizations and quality improvement initiatives.
  • May supervise staff and workflow of the department in some roles.
  • Interacts with all levels of staff in various departments, physicians, patients, families and external contacts to clarify information and provide support.
  • Minimum Qualifications

    Must possess a current, valid RN license in the state of practice or a temporary/interim RN license. BLS certification is required. Additional certification or continuing education may be needed based on area of practice. Experience in the clinical area for which the position is intended is desired; a minimum of one year of experience in an acute care hospital setting within the past 12 months is required.

    Preferred Qualifications

    Bachelor’s degree preferred. Professional certification preferred. Additional related education and/or experience preferred.
