Registered Nurse RN Home Care East Valley

Overview

Banner Home Health is a four‑star home care agency located in Mesa, Arizona. Our Home Health Registered Nurse provides direct patient care and supports interdisciplinary teams. Nurses enjoy an autonomous practice within patients’ homes, with support from a dedicated team of preceptors, schedulers, IT specialists, QI personnel, telehealth resources, clinical managers, and triage staff.

Schedule: Training Core:
Monday‑Friday 8:00am–5:00pm. After training, options include full‑time (40 or 32hr/week), part‑time (20–28hr/week), or BSS/PRN (minimum 2 shifts/month) with flexible weekday and weekend availability.
Location: East Valley – POD 1 (East Mesa, Gold Canyon, Apache Junction, Superior), POD 2 (Mesa, Gilbert, San Tan Valley, Queen Creek, Florence), POD 3 (NE Mesa), POD 6 (Casa Grande, Arizona City, Eloy, Coolidge, Maricopa), POD 8 (Phoenix, Mesa, Tempe, Chandler), POD 9 (Phoenix, Scottsdale, Fountain Hills, Cave Creek, Carefree).

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs; review history, diagnostics, and labs; report abnormal results to physicians.
• Formulate and revise individualized plans of care, including discharge plans, based on assessment data and patient/family input.
• Implement the plan of care through direct patient care, coordination, delegation, and supervision of the healthcare team.
• Evaluate patient and family response to care and teaching; revise the plan of care as needed.
• Document assessment, planning, implementation, and evaluation in the patient record; ensure legible, timely documentation.
• Provide evidence‑based care; participate in research activities and shared leadership; mentor new staff when appropriate.
• Contribute to patient outcomes through efficient, safe, and equitable care and participate in quality improvement initiatives.
• Supervise staff and workflow in some roles.
• Collaborate with physicians, patients, families, external contacts, and occupational support such as clergy for spiritual care.

• Valid RN license in Arizona (or temporary/interim/compact license). BLS certification required.
• Current: one‑year experience in an acute‑care hospital setting (or physician practice) within the last 12 months.
• In a Home Health setting, ability to operate a personal vehicle, valid driver’s license, and active auto insurance.
• Arizona Fingerprint Clearance Card for all Banner Home Care employees.
• Arizona Criminal History Affidavit signed upon hire.
• Maintain clinical performance competencies and annual validations.

• Bachelor’s degree.
• Professional certification.
• Additional related education and/or experience.
