Licensed Practical Nurse – Palliative Care (Part‑time) in Mesa, AZ role offers flexible scheduling and coverage areas within a home‑care and other patient settings environment.
Responsibilities- Coordinate and provide home visits for palliative care patients, managing symptoms and delivering education.
- Document assessments, patient status, and outcome goals in the EMR after each visit.
- Ensure regulatory compliance with documentation requirements and maintain HIPAA confidentiality.
- Identify patient needs and initiate appropriate goals, updating them as needed.
- Communicate and coordinate consistently with interdisciplinary team members, other healthcare providers, and insurance case managers.
- Act as a liaison and advocate for palliative care patients admitted to the hospital.
- Participate in quality improvement studies and other palliative care initiatives.
- Ensure the plan of care for assigned patients is adhered to and keep the team informed.
- Attend interdisciplinary team meetings, providing patient updates.
- Participate in scheduled staff meetings and in‑service training sessions.
- Perform additional duties as agreed upon to meet agency needs.
- RN or LPN with a minimum of 3 years of full‑time equivalent experience.
- Past nursing experience should be in a transferable or applicable clinical venue.
- Currently licensed as a Registered or Practical Nurse to practice in Arizona.
- Demonstrated clinical competency and ability to collaborate with others.
- Ability to initiate and plan own work activities, effective time‑management skills, and professionalism.
- Good physical stamina and mental health to perform nursing duties.
- Valid Arizona nursing license or a valid compact state license for up to 30 days until an Arizona license is issued.
- Responsible, safe driving to and from clients’ residences.
- Effective communication with patients, families, and the care team.
- Ability to lift up to 30 pounds and perform intermittent physical activities required for patient care.
- Maintain professional appearance and adhere to agency dress code.
- Support agency’s code of conduct, standards of practice, and patient rights.
- Document care within appropriate timelines using agency forms and submit to agency as directed.
- Participate in periodic auditing and quality assurance activities.
