Position Summary
Home Health Registered Nurse – East Valley – POD 6 (Primary cities: Casa Grande, Eloy, Coolidge, Maricopa). The role involves providing autonomous, expert nursing care in patients’ homes, focusing on medical intervention and building relationships with patients and caregivers. The RN will work a minimum of two shifts per month, with a flexible schedule that varies from week to week after a 30‑60 day training period.
CoreResponsibilities
- Assess the patient’s physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs; review history, diagnostics, and laboratory data; complete assessments and reassessments as needed.
- Formulate and revise a patient‑specific plan of care, including discharge plans, with realistic and measurable goals developed in collaboration with patient, family, and the multidisciplinary team.
- Implement the plan of care through direct patient care, coordination, delegation, and supervision of the health‑care team, ensuring compliance with policies, procedures, and regulatory requirements.
- Evaluate patient and family response to care and teaching; revise the plan of care to reflect changing needs and progress toward goals.
- Document assessment, planning, implementation, and evaluation in the patient record, ensuring legibility, timeliness, and accuracy.
- Provide care based on best evidence, participate in research activities, shared leadership, and the education and orientation of personnel, patients, families, and visitors.
- Contribute to society by improving national nursing quality indicators and engaging in professional organizations.
- Supervise staff and workflow in some roles as required.
- Interact with patients, families, staff, physicians, and external contacts, including clergy for spiritual support, to share information and clarify care plans.
- Current, valid RN license in the state of practice (or temporary/interim/compact license).
- BLS certification required.
- One year of experience in an acute‑care hospital setting, or in a physician practice/acute‑care setting, or in a Home Health setting (within the past 12 months).
- Ability to operate a personal vehicle, possess a valid driver’s license, and maintain active auto insurance (for Home Health positions).
- Arizona fingerprint clearance card and criminal history affidavit required for Behavioral Health roles and for Banner Home Care.
