Gentiva Hospice is hiring an After hours RN Care Manager to provide high-quality hospice care outside standard hours. The role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours.
The RN will assess needs, coordinate care, and serve on the IDG to ensure coordination among physicians, families, and the hospice team. Requires an active RN license and relevant hospice/home health experience.
