Nurse Practitioner (NP) / Physician Assistant (PA) - Post-Acute, East Valley, AZ

Location:

Mesa, AZ (Primary) | Some coverage in Chandler

Setting:

Skilled Nursing & Assisted Living Facilities

Schedule:

Full-time, weekdays on-site9 AM – 5 PM, plusweekend rotation once or twice per month

We’re hiring a full-time Nurse Practitioner or Physician Assistant to provide ongoing care within post-acute and long-term care settings across

Mesa and Chandler.

This position offers steady weekday hours, autonomy, and a manageable patient load in a supportive clinical environment.

Providers typically see
15–16 patients per dayin Skilled Nursing and Assisted Living communities, focusing on established, stable residents and new admissions as needed.

Key Responsibilities
  • Provide direct, in-person care at assigned SNF and ALF facilities
  • Maintain anon-site presencedaily
  • Participate inweekend rotationonce or twice per month
  • Manage 15–16 patient visits per day, including new and follow-up appointments
  • Respond to nursing staff calls related to your assigned patients
  • Conduct assessments, adjust medications, and manage chronic conditions
  • Collaborate with facility staff and interdisciplinary teams
  • Document all encounters accurately usingPoint Click Care  (PCC)
    EMR
Qualifications
  • Active, unrestricted

    Arizona NP or PA license
  • Graduate of an accredited U.S.

    -based NP or PA program
  • Preferred:1–2 years of post-acute, long-term, or primary care experience
  • Strong communication, clinical reasoning, and organizational skills
  • Reliable, professional, and comfortable working independently
