$140,0.00 - $150,000 a year (based on experience)

- The Provider works in a field-based capacity completing visits to patient homes for Chronic Kidney Disease / Primary Care needs. Providers also have telehealth opportunity based on patient need and location
- Job duties include complete history and physical, creation of treatment plan, order of diagnostics and lab testing, prescribing of medications, routine team meetings, patient education, care plan assessments, documentation of services
- Licensed Nurse Practitioner in the state of Arizona
- Reliable Transportation
