JOB SUMMARY
The Case Manager provides case management services to students who are currently enrolled in the program or completed the internship program and are in the follow-up phase.
Responsibilities include:
to provide guidance, employment and educational support. Also, assist team with recruitment, pre-screening, onboarding and supervision of teens attending the academy and career internship placement. Responsible for sharing a case load (Alumni) of up to 25 with the Data & Outcome Manager to provide support post program for up to a year. Responsible for keeping records and notes utilizing the Kidtrax system.
Assist Data & Outcome Manager with any reporting needed for meetings and grant proposals. Develops and maintains community partnerships and supports AZ Youth force programming including events and connections with employers and/or schools. Carries a caseload of 20 to 40 students and maintains accurate records and notes for each student
- Assists interns with development of career/employment plan
- Collaborates with Data & Outcome Manager to provide support post internship
- Promotes program within the community (companies, CBO’s and schools)
- Manages an active case load of up to 20 interns and alumni up to 25
- Maintains appropriate case notes and documents in system and hard copy file
- Develops and maintains community partners such as schools and CBO’s
- Assists with other activities such as career fairs/job fairs
- Assists with interns’ interviews
- Maintains communication with interns and placement managers
- Provides interns with community resources as needed
- Assists with outreach and presentations as needed
- High school diploma or equivalency
- 1-2 years’ experience in case management
- 1 year experience working with youth ages 14-21
- Must have and maintain a valid Arizona driver’s license
- Ability to maintain and cultivate excellent community relationships and possess excellent communication and customer service skills
- Ability to interact effectively and communicate (in writing and orally) with a variety of members, local government entities, neighborhood groups, agencies, schools, volunteers and teens
- Must have the ability to effectively communicate with and maintain productive and effective working relationships with organization staff and management in accomplishing goals and objectives
- Ability to represent and make presentations to students, clubs, local partner agencies and employers as needed
- Proficient skills in the use of Microsoft Office software, including Outlook, Word, and Excel
- Able to react to change productively, work flexible schedule when needed and handle other essential and marginal functions as assigned
- Familiar with social media platforms
- Plan, organize, implement new ideas
- Travel between BGCAZ Branches, schools and community sites is required. Must meet the insurance standards of BGC.
- Work is performed in Branches, the community as well as an office setting
- Evening or weekend hours required on an as needed basis
