- The Admissions Navigator serves as a critical bridge between community partners, prospective clients, and North Sight Recovery’s internal care teams. This hybrid role is primarily responsible for client navigation, transportation coordination, and intake support, while also serving as backup support to the Admissions Coordinator during periods of downtime or high call volume.
The Admissions Navigator plays a key role in ensuring timely, compassionate, and safe transitions for individuals stepping down from community partners or entering care through new intake pathways. This position enhances continuity of care while supporting admissions flow, access, and responsiveness.
- The Admissions Navigator’s primary responsibility is to support clients through safe, timely, and coordinated navigation services across care settings and community partners.
- Review upcoming appointments for assigned shifts and proactively communicate with medical providers, case management staff, and clients to ensure readiness and alignment.
- Prepare, gather, and review all required documentation related to scheduled appointments, intakes, or services.
- Provide transportation for clients to and from medical, behavioral health, government, and community-based appointments, ensuring safety, professionalism, and compliance with program requirements.
- Build and sustain effective relationships with community partners to support coordinated care and medical service continuity.
- Communicate and collaborate as needed with external stakeholders, including law enforcement, probation officers, governmental agencies, nonprofit organizations, and community partners.
- Serve as a knowledgeable guide for clients, applying an understanding of systems navigation, including healthcare and criminal justice processes, to reduce barriers to care.
- Maintain consistent communication with the care coordination team when downtime is available to support additional navigation needs.
- Accurately and timely document all client interactions, transportation activities, and services provided in the electronic medical record (EMR).
During periods of downtime, high call volume, or coverage needs, the Admissions Navigator may provide backup support to the Admissions team.
- Support admissions workflow during periods of downtime, staff coverage needs, or elevated call volume.
- Answer inbound phone calls in a professional, empathetic, and timely manner.
- Assist with basic intake support and prescreen information gathering, ensuring accurate routing to Admissions Coordinators or Clinical Intake Specialist.
- Support admissions workflow by relaying messages, scheduling follow-ups, and coordinating transportation related to intake needs.
- Maintain confidentiality and comply with HIPAA, 42 CFR Part 2, and organizational policies at all times.
