Specialty Case Manager
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Job SummaryThe Specialty Case Manager provides case management services in the community to individuals who have a mental health and/or substance use diagnosis, survivors of domestic violence, and/or those considered high‑risk due to their acuity level. The role includes planning, connecting, advocacy, coordination, and monitoring to help patients gain access to health services, financial assistance, housing, employment, education, social services, and natural supports.
The manager assesses needs, develops and monitors individualized treatment plans, and documents services accurately and timely. The Specialty Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients while implementing advanced techniques and interventions for high‑risk individuals.
- High school diploma or GED required. Associate’s degree or higher in a field related to behavioral health preferred.
- Recovery from alcohol or drugs and/or GMH for equal to or greater than one consecutive year preferred.
- 1–3 years of experience in a behavioral health position providing outreach and engagement activities preferred.
- 1–3 years of experience working with special populations (individuals with serious mental illness, survivors of domestic violence, high‑cost/high‑need clients) preferred.
- Or equivalent combination of education and experience.
- Current AZ driver’s license (valid and in good standing).
- 39‑month MVR.
- Arizona Fingerprint Clearance card.
- Jail clearance (if applicable).
