Job Summary:
The Case Manager provides trauma-informed case management and supportive services to individuals experiencing homelessness and related behavioral health challenges. This role conducts client assessments, develops service plans, coordinates resources and referrals, maintains required documentation, and supports clients in working toward housing stability and individualized goals. The Case Manager works collaboratively with clients, staff, and community partners to support a safe and client-centered environment.
Responsibilities:
- Provide case management services and support to clients participating in shelter and housing programs.
- Conduct intake screenings, assessments, orientations, and service planning for program participants.
- Assess client needs and coordinate referrals, resources, transportation, appointments, and supportive services as appropriate.
- Develop, implement, monitor, and update individualized, strength-based service plans and client goals.
- Maintain ongoing contact and engagement with assigned clients to support program participation, service coordination, and progress toward individualized goals.
- Support trauma-informed, client-centered service delivery practices that promote safety, dignity, and appropriate client engagement.
- Assist clients with accessing housing resources, community services, benefits, behavioral health support, employment resources, and other supportive services.
- Coordinate discharge planning, transition services, follow-up support, and referrals as appropriate.
- Complete and maintain accurate, timely, and thorough client documentation, case notes, HMIS data entry, assessments, and program records in accordance with agency and program requirements.
- Maintain client demographic information, program data, and reporting requirements.
- Participate in case management meetings, staff meetings, trainings, supervision, and other required meetings or activities.
- Coordinate and collaborate with internal departments, community partners, service providers, and external agencies to support client and program needs.
- Recognize signs and symptoms of mental health concerns, substance use, trauma, or crisis situations and coordinate referrals or support services as appropriate.
- Maintain professionalism, confidentiality, ethical conduct, and appropriate boundaries in all interactions with clients, staff, community partners, and the public.
- Support a safe, clean, and respectful environment for clients, staff, and visitors.
- Participate in required trainings and professional development activities.
- Support program operations and administrative needs as assigned.
- Perform other related duties as assigned.
Qualifications:
Education:
- High School Diploma or GED required.
- Associate’s or Bachelor’s degree in Social Work, Human Services, Behavioral Health, Psychology, or a related field preferred.
- Equivalent combinations of education and relevant experience may be considered.
Experience:
- Minimum one (1) year of experience providing case management, client services, or supportive services required.
- Experience working with individuals experiencing homelessness, serious mental illness (SMI), substance use disorders, trauma, or related behavioral health challenges preferred.
