Suvida Healthcare is seeking a Care Coordinator to support high‑risk patients within our Chronic Care Program. You will work closely with the Nurse Care Manager to conduct daily follow-ups, process DME and home health orders, triage calls, and retrieve hospital records, ensuring timely, patient‑centered care.
The ideal candidate has MA or LPN/LVN background, 3–5 years of relevant experience, strong communication, and bilingual English/Spanish abilities to serve our Medicare-eligible Hispanic
