• Conduct comprehensive assessments of patients to identify their unique needs and develop individualized care plans
• Develop, implement, and evaluate care plans in collaboration with the interdisciplinary care team
• Provide direct patient care, including but not limited to administering medications, performing treatments, and managing medical conditions
• Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care
• Educate patients and their families on disease management, medication usage, and other vital health information
• Document patient information accurately and timely in accordance with company policies and procedures
• Participate in quality improvement initiatives to continually improve patient care and outcomes
• Maintain HIPAA compliance and adhere to company policies and procedures

• Current licensure as an LVN or RN in the state of Texas
• Bachelor's degree in Nursing (BACHELORS)
• 0 years of minimum experience (MINIMUM YEARS OF EXPERIENCE)
• Ability to work full-time schedule ()
