About the Role
We’re seeking a Registered Dietitian Diabetes Educator to provide individualized and group diabetes education, support patient goal‑setting, and help individuals manage Type 1, Type 2, and Gestational Diabetes. You’ll work closely with a multidisciplinary care team and play a key role in improving patient health outcomes across diverse communities.
Key Responsibilities- Deliver diabetes self‑management education following ADA standards
- Support patients in creating and achieving health goals
- Assess learning and care barriers to customize education
- Collaborate with providers and care teams to improve outcomes
- Conduct group classes and individual assessments
- Participate in quality improvement initiatives
- Travel between clinical sites as needed
- Registered Dietitian with active state license
- Certified Diabetes Educator (CDE) required
- Experience with insulin pumps and CGMs preferred
