Serves as a key member of the PACE Interdisciplinary Team, contributing to care planning, implementation, and evaluation to support frail elders in maintaining community living safely. Provides direct nursing care, conducts assessments, develops and updates care plans, and ensures compliance with quality standards.

Key Responsibilities- Deliver direct patient care, perform assessments, and develop personalized care plans.
- Gather medical and social information to inform safe decision-making.
- Monitor care delivery, ensure adherence to policies, and evaluate quality assurance.
- Recognize health and social needs, prioritize issues, and coordinate referrals and follow-up.
- Collaborate with the interdisciplinary team to create comprehensive care strategies, including participant and family input.
- Assist in determining Nursing Home Level of Care and ensure diagnostic/testing compliance.
- Manage medication processes, including ordering, administering, and monitoring adherence, and coordinate home delivery of supplies.
- Educate and counsel participants and caregivers on care plans and treatments.
- Maintain communication with primary care providers and team members, fostering strong relationships with participants and community providers.
- Participate in after-hours visits and pass a fit test for respirator use.
- Attend meetings, train staff, oversee team workflow, and generate performance reports.
- Provide leadership, guidance, and problem-solving support to the team.
- Licensed Registered Nurse in the Commonwealth of MA.
- At least two years of experience with geriatric patients.
- Proficient in Microsoft Office tools.
- Ability to travel locally using a personal vehicle.
- Strong clinical judgment to identify and address issues promptly.
- COVID vaccination preferred.
