Social Worker, Crisis Counselor

Element Care seeks a compassionate and dedicated Social Worker to join our team in the Social Services department, providing specialized support within our Program of All-Inclusive Care for the Elderly (PACE). This role is critical in assisting elderly clients to live a dignified and independent life in the comfort of their own homes, coordinating healthcare and supportive services tailored to each individual's needs.

Responsibilities

• Conduct comprehensive assessments of client's social, emotional, and environmental needs.
• Formulate and implement care plans in conjunction with interdisciplinary teams.
• Provide counseling and support to clients and their families on coping with cognitive and aging-related challenges.
• Advocate for clients' access to community resources and services, ensuring their needs are adequately met.
• Maintain accurate records and documentation in compliance with regulatory bodies and funding sources.
• Participate in team meetings and contribute to the continuous evaluation and improvement of services provided.
• Offer crisis intervention services as needed and coordinate with other professionals to provide seamless care.
