The Patient Navigator is a vital role in delivering high-quality patient engagement and customer service for our Federally Qualified Health Center (FQHC). This phone-based position focuses on both outbound and inbound communication to educate patients about their assignment to CFCE, the comprehensive services we offer, and how to access care.

Some of the Functions Include:
  • Conduct outbound calls to patients listed on capitation rosters to inform them of their assignment to CFCE.
  • Educate patients on available services, including primary care, dental, behavioral health, and specialty care.
  • Guide new patients through the onboarding process and direct them to appropriate departments for scheduling.
  • Follow up with patients regarding the Sliding Fee Discount Program, including eligibility, applications, and benefits.
  • Identify barriers to accessing care and connect patients with appropriate internal or community resources.
  • Screen patients for social determinants of health, including housing, food insecurity, and transportation needs.
  • Collaborate with front office and clinical teams to coordinate appointments and ensure appropriate referrals.
  • Provide exceptional customer service while addressing concerns and routing inquiries to the appropriate departments.
  • Maintain accurate documentation of outreach, referrals, and patient interactions in the EHR and call-tracking system.
  • Maintain patient confidentiality and comply with HIPAA and organizational policies.
  • Participate in team meetings, training, and other assigned duties as needed.
  • Perform other duties as assigned.
  • High school diploma required;
    Associate’s degree in Social Work, Public Health, Health Administration, Human Services, or a related field preferred.
  • Minimum of 2 years of experience in a Community Health Center or Federally Qualified Health Center (FQHC) setting, patient outreach, case management, or community health work preferred.
  • Florida Community Health Worker (CHW) Certification preferred, or willingness to obtain within 6 months of hire.
  • Must pass a Level 2 background screening in accordance with Florida Statute and AHCA requirements.
Skills Needed
  • Strong verbal communication, active listening, and customer service skills.
  • Ability to manage a high volume of calls with professionalism, empathy, and efficiency.
  • Proficiency with electronic health record (EHR) systems and accurate documentation.
  • Strong organizational, time management, and attention-to-detail skills.
  • Ability to identify social determinants of health and connect patients with appropriate resources.
  • Cultural competency and sensitivity when working with diverse patient populations.
  • Knowledge of trauma-informed, non-judgmental communication and behavioral health terminology.
  • Bilingual in English/Spanish or English/Haitian Creole strongly preferred.
  • Familiarity with community health models and sliding fee programs preferred.
