Position Summary

This professional position serves as the regional incident management consultant. The incumbent independently performs multiple functions related to incident management, ensuring CMS compliance with provider reporting of critical and reportable incidents within required time frames, follow‑up on Data Analysis team needs, documenting trends, and ensuring incident reports are obtained for emergency room visits. The role also includes presenting data and information at quarterly provider regional meetings.

Key Responsibilities
  • Ensure incident reports from providers are correctly classified and complete before entering the Agency’s Incident Management System (IMS).
  • Follow up with providers to collect missing information; ensure follow‑up reports are submitted within the required five‑day timeframe and are entered into IMS, closing incidents.
  • Analyze monthly DCF Intake data, verify critical incidents, ensure wellness visits, review closed allegations for trends, enter progress notes into IMS, and submit intakes for closure.
  • Conduct follow‑up activities for providers and clients identified by the Data Analysis Team, assess health and safety needs, determine technical assistance or remediation, and record activities in SharePoint.
  • Review ER visit reports, work with non‑compliant providers to provide timely incident reports, and ensure remediation activities.
  • Collaborate with the regional trainer to develop training curricula focusing on incident reduction and best practices; present training at quarterly regional meetings.
  • Attend meetings, conference calls, training, workshops, and special projects, ensuring timely completion and compliance with deadlines.
Minimum Qualifications
  • High School Diploma or equivalent.
  • Experience with public speaking.
  • Proficient use of Microsoft applications (Excel, Word, PowerPoint, SharePoint).
  • Two years of professional experience working with people with disabilities or in the health and human services field.
