The Work You Will Do
The primary function of the Health Facility Evaluator II is to conduct inspections utilizing an established survey process.
The survey process includes evaluating health care facilities for state licensure and federal certification requirements for participation under Medicare/Medicaid.
The incumbent documents findings and completes written reports of survey findings.
The incumbent may be required to perform surveys as an individual, team member, or team leader.
As assigned by management, this position requires successful completion of the Surveyor Minimum Qualifications Test within the first 12 months of employment as specified by law.
The position is mission essential, may require weekend or holiday work, and may involve emergency duties during disasters or emergencies, possibly in another county.
Travel may be extensive with significant overnight travel.
RequirementsAll candidates must successfully complete a criminal background investigation as a condition of employment.
Proficiency with computer equipment and software programs, such as Microsoft Word and Outlook, and smartphone technology.
Strong attendance record; available to work before and after normal work hours, including weekends and holidays.
Knowledge, Skills, and AbilitiesKnowledge of state licensure and federal certification survey requirements; methods of compiling, organizing, and analyzing data.
Ability to develop and implement monitoring programs; understand and apply rules, regulations, policies, and procedures; prioritize workload; plan, organize, and coordinate work assignments; establish and maintain effective working relationships; communicate effectively; oral communication, listening, interview procedures, team dynamics, performance evaluation techniques.
Proficiency with microcomputer equipment and software for data manipulation; ability to travel; ability to lift up to 25 pounds.
Minimum QualificationsThree (3) years professional or nonprofessional health care licensure, regulation, and/or certification experience is required.

The policy requires successful completion of State and National criminal history check before employment. Background screening includes fingerprinting for State and Federal criminal records checks through the Florida Department of Law Enforcement and the Federal Bureau of Investigation and may include local checks.
