Summary

Clinical Practice Pharmacist performs a variety of pharmacy procedures requiring professional knowledge and skills in a large and complex department of pharmacy. Emphasis is on compounding and dispensing a variety of medications, manufacturing complex solutions, and supervising work of pharmacists not registered within the state. All work is performed in accordance with recognized professional standards, hospital and pharmacy rules and regulations, and applicable state and federal laws.

Responsibilities
  • Integrate drug distribution and clinical pharmacy services in the pharmaceutical care model to assure safety and efficacy, and reduce costs.
  • Evaluate physicians’ orders, prescriptions, vouchers, and cart fill lists for appropriateness of therapy, taking into account drug interactions, allergies, and relevant patient conditions.
  • Assist in the collection of data for drug utilization evaluations.
  • Provide drug therapy monitoring and intervention through target drug programs depending on area assignment.
  • Serve as a role model and educator for pharmacy students during internships and externships, and pharmacy residents at JHS as required.
  • Participate in topic discussions with clinical clerkship students when requested.
  • Provide in‑services to nursing and/or other health care professionals.
  • Actively participate in staff development programs, providing programs when requested.
  • Provide therapeutic drug level monitoring (e.g., recognize need for levels, schedule and interpret levels).
  • Report adverse drug reactions encountered by assigned patients.
  • Demonstrate behaviors of service excellence and CARE values (Compassion, Accountability, Respect, Expertise).
  • Perform other related duties as required.
Experience

Generally requires 5 to 7 years of related experience.

Education

Pharm.

D. degree or B.S. with equivalent clinical knowledge is required.

Management competencies: knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources;

skill in monitoring/assessing performance to make improvements or take corrective action; skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions and make conclusions or approaches to problems; ability to plan, implement, and evaluate programs; ability to establish goals and objectives; ability to recognize, analyze, and solve a variety of problems. Strong patient care orientation, self‑motivation, and good interpersonal, communication, and problem‑solving skills and the ability to work effectively as a team player are required.

Credentials
  • Valid Florida pharmacy license or certification as required.
Working Conditions

Physical Requirements

Job function requires extended standing or walking. Must be able to carry objects weighing 21 to 50 pounds. Able to lift or move objects weighing 51 to 100 pounds with the use of appropriate devices. Jobs in this group require frequent visual acuity to perform activities related to extended use of computers. Environmental

Conditions

Jobs in this group are required to function in a fast‑paced environment with occasional high pressure or emergent and stressful situations. Frequent interaction with a diverse population including team members, providers, patients, insurance companies and other members of the public. Function is subject to indoor environmental conditions, with occasional outdoor exposures. Exposure to communicable diseases, toxic substances, medicinal preparations and other conditions common to a hospital and medical office environment.

May also be exposed to needle sticks, airborne infections, medical gases, X‑Ray, chemical exposures and other potential hazards. Must wear Personal Protective Equipment (PPE) when exposed to infectious/clinical hospital environment.
