PositionSummary
Clinical Hospital Pharmacist by using established policies and procedures is responsible for: drug therapy of assigned patients; provision of clinical drug information to meet the needs of the physician, nurse, and patient; education of pharmacy students, residents, and interns; and distribution of medications.
Duties & Responsibilities- Evaluates medication therapy for appropriateness, considering patient conditions, allergies, renal status, and other disease states; assesses safety and potential efficacy of prescribed medication including identity, strength, dose, frequency, duration, and directions of use; reviews formulary status and availability; considers drug-drug or drug-food interactions; and evaluates potential alternative therapy (less costly or therapeutically superior).
- Provides therapeutic drug level monitoring when appropriate.
- Communicates with nurses and prescribers when errors are detected or to optimize therapy.
- Consults with other practitioners as appropriate (e.g., other Clinical Hospital Pharmacists, Pharmacy Management, Attending Physician).
- Identifies trends during routine pharmaceutical care, collects data, and formulates practice recommendations to improve outcomes or reduce cost.
- Presents data to the appropriate committee as required.
- Receives exceptional feedback from medical staff regarding the pharmacist's impact on patient outcomes.
- Performs formal, written consults and documents all relevant clinical consult information in the medical record.
- Demonstrates accuracy in dispensing, preparing, labeling, and operating automated dispensing machines and entering data accurately.
- Identifies ways to decrease medication dispensing errors.
- Completes tasks with appropriate speed without sacrificing accuracy.
- Demonstrates reliability and a conscientious, thorough, and complete approach to work.
- Shows initiative, communicates effectively, and collaborates with teammates; adapts to changes in work assignments and schedules for patient care or departmental needs.
- Coordinates tasks to minimize non-essential interruptions and meets deadlines.
- Communicates limitations or needs to supervisors and follows up as appropriate; seeks clarification as needed.
- Demonstrates problem-solving skills and performs routine duties without direct supervision; brings concerns to supervisors with solutions.
- Maintains good judgment and independently applies policies while making reasonable exceptions when needed for patient safety.
- Contributes to process improvements and participates in quality initiatives, cost containment, and patient outcomes improvements.
- Develops the skills of others and may be involved in orientation and training of staff, students, and healthcare personnel.
- Assists in coaching and performance development; meets area-specific quantitative requirements and receives satisfactory evaluations.
- Participates in teaching activities and publications as appropriate; presents at meetings or in-service sessions; authorship in publications as applicable.
- Active participation in multidisciplinary administrative or clinical committees.
- Proficient with area-specific technology and techniques (e.g., automated dispensing, computer systems, aseptic technique).
- Identifies opportunities for DUE or Target Drug Program activities and presents them to the appropriate committee; aims for practical, impactful results.
- Takes initiative to develop and implement new services or programs related to productivity, QA, or PI.
- Supervises supportive personnel and ensures efficient operation of the area; optimizes workflow and ensures daily task completion.
- Performs other related duties as required and demonstrates CARE values (Compassion, Accountability, Respect, and Expertise).
Required Qualifications:
Generally requires 5 to 7 years of related experience.
- Pharm.
D. degree and residency training or equivalent proven experience. - Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines.
- Ability to communicate effectively in both oral and written form.
- Possession of or eligible for Florida pharmacist license.
