Address: 160 NW 170th ST, North Miami Beach, FL 33169

Hours for shift: 7:00am to 3:30pm, Rotating weekends every 6 weeks

Summary

Clinical Hospital Pharmacist by using established policies and procedures is responsible for: drug therapy of assigned patients; provision of clinical drug information to meet the needs of the physician, nurse, and patient, education of pharmacy students, residents, and interns; and, distribution of medications.

Responsibilities
  • Evaluates medication therapy for appropriateness, considering relevant patient conditions including age, allergies, renal status, and other disease states.
  • Assesses safety and potential efficacy of the prescribed medication (identity, strength, dose, frequency, duration, and directions).
  • Assesses formulary status and availability of prescribed medications.
  • Considers potential drug–drug or drug–food interactions and likelihood of alternative therapy (less costly or therapeutically superior).
  • Provides therapeutic drug level monitoring when appropriate and communicates with nurse/prescriber when errors are detected or to optimize therapy.
  • Consults with other practitioners as appropriate (e.g., other Clinical Hospital Pharmacists, Pharmacy Management, Attending Physician).
  • Identifies trends during routine pharmaceutical care, collects data, and formulates practice recommendations to improve outcomes or reduce cost.
  • Documents all appropriate clinical consult information in the medical record using standard hospital guidelines.
  • Dispenses, prepares, mixes, labels, and processes medications accurately; operates automated dispensing systems and inputs data correctly.
  • Identifies ways to decrease the possibility of medication dispensing errors.
  • Completes tasks with appropriate speed while maintaining accuracy.
  • Demonstrates reliability and a proactive work ethic; maintains a needs-no-reminder attitude and completes work promptly.
  • Identifies opportunities to improve quality and adapts to workload changes;
    Collaborates with team to support patient care and departmental needs.
  • Demonstrates effective time-management, prioritizes tasks, and meets deadlines.
  • Maintains good communication with supervisors and co-workers; seeks clarification or guidance as appropriate and takes initiative to improve inter-staff communication.
  • Demonstrates problem-solving skills; performs routine duties without direct supervision and offers solutions when concerns arise.
  • Supports staff development and coaching; may participate in orientation, education, and training of pharmacy students, interns, residents, and other personnel.
  • May participate in research or publication activities as described in the original duties (e.g., journals, posters, or internal publications).
Experience

Generally requires 5 to 7 years of related experience.

Education

Pharm.

D. degree and residency training or equivalent proven experience is required.

Skill

Ability to analyze, organize and prioritize work, communicate effectively in oral and written form, handle difficult situations professionally, follow instructions, use technology and software applications, and demonstrate literacy in computerized information systems.

Credentials

Possession of or eligible for Florida pharmacist license is required.

Working Conditions

Physical Requirements - Extended standing or walking; ability to carry 21–50 pounds and, with devices, 51–100 pounds; frequent visual acuity for computer work; reasonable accommodations provided by unit/department in collaboration with RAC.

Environmental Conditions - Fast-paced environment with potential for high pressure or emergent situations; interaction with diverse populations; indoor environment with occasional outdoor exposure; potential exposure to infectious/clinical environments, needle sticks, chemicals, and other hazards; PPE required as appropriate; reasonable accommodations available.
