Shift: Full Time
Salary: $20–$23/hour
The Social Service Assistant supports the planning, organization, implementation, and evaluation of the Social Services Department in alignment with federal, state, and local standards, facility policies, and procedures. This role assists with discharge planning, development and implementation of social care plans, resident assessments, and ensures residents receive safe and comprehensive care throughout their stay and upon discharge.
Key Responsibilities:Interview residents and families to obtain social history.
Participate in discharge planning and coordinate with community resources to ensure safe transitions.
Assist in planning, organizing, and implementing social service programs.
Report any occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals according to facility policy.
Participate in life safety and emergency drills; fulfill responsibilities during activation of the facility’s emergency plan.
Seek out and incorporate new methods and best practices into existing social service programs.
Minimum of 2 year of experience in a Skilled Nursing Facility (SNF) setting.
Strong customer service experience working with elderly populations.
Currently working toward a degree in Social Work or a related field (preferred).
MEDICAID experience in an LTC or SNF setting highly preferred.
Previous social service experience and education a plus.
