Summary
Pharmacy Resident PGY1 provides, to the patients of the hospital, accurate, safe, efficient and cost effective drug distribution and pharmaceutical services, either directly or indirectly, through the supervised efforts of supportive staff. The resident participates in a departmental training program designed to develop the knowledge and skills needed to provide pharmaceutical care including the provision of rational pharmacotherapy and the delivery of appropriate product related services.
Responsibilities
Monitors drug therapy for assigned patients (taking into account age and patient specific considerations) and intervenes when appropriate to optimize therapeutic outcomes, minimizing adverse drug events and reduce costs. Provides consultative services as requested by medical staff, taking into account patient specific factors such as patient age and clinical status. Depending on assigned location, actively participates in patient care rounds with the medical staff and/or interdisciplinary rounds.
Provides drug information services for physicians, nurses and other health care personnel including primary literature review and evaluation. Response takes into account patient specific factors such as age group and clinical status. Provides therapeutic drug monitoring and pharmacokinetic dosing for selected medications with narrow therapeutic indexes, taking into account all pertinent parameters such as developmental pharmacokinetics, age related alterations, organ function, weight.
Participates in formulary management by evaluating non-formulary requests, restricted medication orders, target drug programs (i.e. iv to po) and either offers alternative recommendations or makes a clinical decision for exceptions when applicable. Assists in the tailoring of pharmacy service to the specific needs of the patient population serviced. Troubleshoots drug distribution concerns and serves as a liaison between the pharmacy and the patient, medical and nursing staff.
Participates in Task Forces, Project Teams, Committees and special projects as assigned. Provides clinical and distributive productivity data, as requested. The resident submits a monthly clinical activities report to the residency program director. Reports, investigates, and assist in the formulation of corrective action (when appropriate) for adverse drug reactions and medication errors, including near misses. Participates in the development of criteria, data collection forms, analysis, formulation of recommendation for improvement and presentation of drug use evaluations Writes articles for departmental newsletters and other hospital publications when requested Actively participates in departmental staff development and continuing education programs including case presentations, journal clubs and other departmental education programs.
Participates in Departmental or hospital-wide performance improvement activities as assigned. Initiates projects that will lead to continuous improvement of services provided and/or optimization of drug therapy utilization. Conducts patient medication histories and/or discharge consultations for selected inpatients. Interviewing techniques are tailored to the appropriate age group and cognitive level of the patient/caretaker. Provides on-going informal drug therapy education for the medical, nursing and allied health staff.
Provides in-services and/or formal education programs to nursing, medical, and other health care professionals. Conducts patient medication teaching when requested, assuring that the level of communication is appropriate for the age and education related cognitive level of the patient/care taker. Serves as a role model, preceptor, and educator for pharmacy students, residents and junior staff, as assigned. Participates in academic activities that lead to presentations at local, national, and/or international professional meetings.
Participates in professional association activities, meetings, seminars, and committees, keeping abreast of new developments in the provision of comprehensive pharmacy services. Using knowledge of the principles of growth and development over the patient's life span, assesses data reflective of the patient's status, and interprets the appropriate information needed to identify each patient's age specific drug therapy requirements. Develops and completes a residency project.
Attends departmental team and staff meetings. Meets with the residency program director at the beginning of the program to evaluate ones background experiences and program expectations. Arranges and attends meetings periodically with the assigned preceptor to evaluate the progress in residency rotations.
