About the role:

As a Care Advocate, you will support Medicare members as they navigate complex health and social needs. You will serve as a trusted guide, helping members overcome barriers to care and access essential community resources. This role is ideal for someone with a strong foundation in community health work, who is deeply empathetic, exceptionally organized, and ready to contribute to building innovative solutions.

You will be part of the Community, a new business within our client operating as a dynamic, fast-paced early-stage startup. This means contributing to care delivery while providing feedback to shape systems and workflows. Your insights working with patients will directly inform our growth and improvements.

While 90% of your work will be virtual, you will also engage in on-site visits in the community two to three times per month.

Key Responsibilities:
  • Build strong, trusting relationships with Medicare members, grounded in empathy, respect, and patience.
  • Communicate with members through inbound and outbound calls, and member inboxes. Member interactions may occur via video calls, audio-only calls, text messages, and emails.
  • Connect members to community-based social services (such as food assistance, transportation, housing support) to address health-related social needs.
  • Support appointment scheduling and healthcare access, including provider lookups and reminders.
  • Help members become stronger self-advocates in managing their health and care.
  • Participate in a flexible schedule including nights and holidays for immediate response to emergency-level cases.
  • Compensation and Benefits:

    Compensation for this role at Vaco/Highspring depends on factors such as skills, experience, licensure, location, and organizational needs. The salary range is provided in this job posting where legally required. Employees may also be eligible for discretionary bonuses, and benefits include medical, dental, vision insurance, and participation in the company's 401(k) retirement plan.
