Care Advocate

As a Care Advocate, you will support Medicare members as they navigate complex health and social needs. You will serve as a trusted guide, helping members overcome barriers to care and access essential community resources. This role is ideal for someone with a strong foundation in community health work, who is deeply empathetic, exceptionally organized, and ready to contribute to building innovative solutions.

You will be part of the Community, a new business within our client operating as a dynamic, fast-paced early-stage startup. This means contributing to care delivery while providing feedback to shape systems and workflows. Your insights working with patients will directly inform our growth and improvements.

While 90% of your work will be virtual, you will also engage in on-site visits in the community two to three times per month.

Compensation for this role at Vaco/Highspring depends on factors such as skills, experience, licensure, location, and organizational needs. The salary range is provided in this job posting where legally required.
