Job Title

Health Information Technician II

Job Summary

Performs quantitative record analysis and ensures completion of medical records by physicians and other departments.

Essential Job Functions
  • Long‑term care, hospital, and SNF charts are audited and analyzed according to department guidelines; incomplete charts are filed in the Incomplete Area to ensure availability for staff in charge of record completion; deficiencies are entered into the Medi Tech System per discipline to be completed; keep Director/Manager informed of all issues pertaining to record completion.
  • Picks up all discharged medical records from all units daily.
  • Assists with release‑of‑information activity and filing.
  • Completes telephone requests from other departments, physicians, physician offices, and responds in a timely manner.
  • Assists physician and clinicians with record completion through the Physician Ambassador Program.
  • Performs other support duties for the HIM Department as needed.
  • Picks up medical record thinnings from nursing units and files promptly in appropriate charts.
  • Ensures all transcriptions are signed by physicians before filing (copy stamp to be used if signatures are missing) and mails transcriptions to physicians requesting mailed copies; files all history & physicals, discharge summaries, and X‑rays in a timely manner.
  • Assists in coordinating transcribed reports from Transcription Company and checks for accuracy.
  • Distributes and files all transcription copies daily to Ambulatory Health Center and all nursing units in a timely manner.
  • Participates in department meetings and in‑services as necessary.
Education

High School Diploma or equivalent; associate degree in HIT preferred.

Job Requirements
  • Completed course in Medical Terminology.
Experience
  • At least 1 year of experience with office work in a healthcare setting.

  • Excellent communication, organizational, and interpersonal skills.
  • Knowledge of medical terminology.
