DescriptionThe Housing Case Manager is responsible for assessing and coordinating Housing Opportunities for Persons with AIDS (HOPWA) and other programs for Care Resource clients.
Job Responsibilities- HOPWA and Other Housing Opportunities Coordination:
- Screen and assess new clients for HOPWA services and other housing stability services (e.g., Section 8, Homeless Assistance Programming, other housing services) according to the intake schedule.
- Complete applications for HOPWA programs and other housing programs.
- Determine eligibility for HOPWA programs and other housing programs.
- Identify resources and refer applicants to appropriate housing programs.
- Follow up with clients on housing stability (HOPWA and non‑HOPWA).
- Refer clients to Rapid Rehousing program.
- Caseload Management:
- Maintain an average annual active caseload as assigned by manager.
- Act as liaison to ensure clients are properly referred to services within agency programs or external services to remove barriers to treatment and care.
- Service Planning and Documentation:
- Ensure all documentation is timely, accurate, legible, and clear.
- Empower clients to participate in their treatment planning as needed.
- Maintain progress notes and reviews in client records per agency policy and program guidelines.
- Input client information using electronic data entry according to agency and departmental guidelines.
- Maintain accurate timesheets reflecting time spent working within the program.
- Prepare program reports and records as requested by the supervisor or manager.
- Manage Resources:
- Maintain comprehensive knowledge of community services to align services with individual client needs.
- Support billing through concurrent documentation of services and budget activities as required (e.g., PCIS, Web‑based systems, Provider Enterprise, Case Watch, Service Point, Next Gen, Electronic Health Records).
- Participate in staff training sessions within specified time frames and as required by the agency or funding source.
- Community Involvement:
- Participate in agency developmental activities as requested.
- Safety:
- Ensure proper hand washing per CDC guidelines.
- Understand and perform assigned role in Emergency Code System.
- Understand and perform assigned role in agency’s Continuity of Operations Plan (COOP).

- Physical Requirements: Requires sitting, walking, hearing, talking, occasional driving, stretching, reaching, and standing. Work performed in office, client homes, community agencies, and hospitals.

- Experience:
- Required:
2 years of experience with homeless/social services. - Preferred: 1 year of HIV/AIDS experience.
- Required:
- Education (Preferred):
- Bachelor’s or better in Health Administration.
- Bachelor’s or better in Public Administration.
- Bachelor’s or better in Social Work.
- Travel Required: Yes – own transportation required.
