• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services, and family members.
• Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.

• Minimum of bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist in the state where practicing.
• Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
• Minimum of one year’s work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift 50 pounds.
• Capability to move, lift, or transfer patients involving lifting up to 100 pounds following safety procedures.
• Requires fine motor skills, visual acuity, and ability to walk/stand for extended periods.
• Ability to push/pull, bend/stoop, reach, kneel, and position frequently.
• Health screen may be required prior to assignment and periodically thereafter, depending on local or state regulations to verify physical capability to perform duties.

Rehab Without Walls, an affiliate of Bright Spring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care. It is designed to help people who have experienced a brain injury, spinal cord injury, or stroke/CVA through accident or illness regain functional capabilities.

By providing care in the home, community, workplace, or school, Rehab Without Walls promotes greater health and significantly improves functional skills and independence for patients.
