Ops Human Services Program Consultant II -
The position will coordinate and conduct timely personal interviews and re-interviews of pregnant patients infected with a Sexually Transmitted Disease (STD) as specified for the purpose of educating patients regarding their infections and appropriate medical care; developing plans to locate and refer their sex and needle sharing partners and others at risk for infection to a medical facility for examination and appropriate treatment.
Performs cluster interviews on persons determined to be potential information sources for known cases. Conduct field investigation to bring about a timely disposition for mother, sex partner and baby field records. Acts as hospital/private provider's liaison on positive tests for STDs. Incumbent is expected to always communicate in a courteous and professional manner. Conduct filed investigation to locate all assigned pregnant females infected with Syphilis for a timely testing and treatment and perform a thorough case management on said patients.
Maintains tracking log on all pregnant women with a reactive serology and provides weekly list of open field records to surveillance supervisor. Ascertains serology status of neonate upon mother's delivery and initiates congenital syphilis worksheet for quarterly review. Incumbent will be responsible for abstracting all medical records needed for review from the healthcare provider who provides treatment and services to the pregnant woman with a congenital case.
Investigate cases utilizing the case management tools, review and prepare all documents to be ready for the Case Review Team. All records not limited to pre-natal, labor and delivery, neonate care and STD records. Acts as congenital syphilis liaison. Investigates all positive neonatal and maternal Treponema and non-Treponema lab results to complete the congenital syphilis case investigation worksheet. Verifies diagnosis, treatment history for mother and baby by telephone and/or field visits to hospital/health care provider or record search of medical records.
Field Investigation and collaboration efforts with DIS for treatment and/or interview of the mother. Incumbent will collaborate with Jackson Memorial Hospital Labor and Delivery for all pregnant females needing desensitization when they have allergies to penicillin. Work with DOH Clinicians and OB to obtain all necessary documents, hospital needs for desensitization. This process might require non-traditional hours. The incumbent is required to assist clinic duties and must be able to work collaboratively with clinical and other key staff that plays a role in providing Linkage to Care Services to the clients of Miami Dade County.
Locate and link all clients infected with HIV, to medical treatment services as well as any other services needed. Prepares and maintains timely and accurate records. Document all activities and updating STARS in a timely manner. Acts as hospital liaison on all pregnant females with positive STD tests. Assist surveillance Manager in Lab/Provider visitation for quality assurance and policy presentations in the field as required.
Other job duties as required to ensure smooth running of the Surveillance Unit. Perform all other related duties as assigned.
Required Knowledge, Skills, and Abilities:
Knowledge of methods of compiling, organizing and analyzing data. Ability to conduct fact-finding interviews. Ability to maintain effective working relationships with others. Ability to conduct investigations. Ability to maintain strict confidentiality Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
Qualifications:
Minimum – Experience in case management, medical record review, or public health investigations. Ability to work collaboratively with clinical staff, Disease Intervention Specialists (DIS), and external partners. Preferred – Incumbent in this position is preferred to be bilingual in English and Spanish or English/Creole. Familiarity with case investigation worksheets, case review tools, and public health surveillance systems. Experience collaborating with clinicians on desensitization protocols or similar clinical processes.
