Healthy Steps Program Coordinator
Responsible for delivering Healthy Steps services and coordinating the efforts of all the team to implement the Healthy Steps program and services.

Qualifications:
Education/
Experience:
Bachelors in Health Science related field required with focus on Child Development specifically age 0-5. Experience working with children and families required. Previous exposure or experience to Healthy Steps program preferred.
Licensure / Certification:
Maintain current CPR certification from the American Heart Association
Skills / Ability:
Bi-lingual (English/Spanish) preferred, satisfactory knowledge of Child Development 0-5 required.
Position Responsibilities (This Is A Non-Exempt Position):
- Recruits and enrolls families.
- Meets with families at every well child visit, either with the physician/pediatric nurse practitioner or separately.
- Conducts waiting room activities, as appropriate, during regularly scheduled office times.
- Conducts home visits at recommended intervals.
- Assesses child's developmental progress.
- Prevents/intervenes/manages child development and common behavior problems.
- Identifies quality resources and provides referrals and follow-up as appropriate to foster community connections and develop a working relationship with the contact person at referral agencies.
- Facilitates/leads parent groups and support groups.
- Documents all home visits and well care baby visits accurately and keeps documentation current.
- Provides educational materials to family as scheduled and as needed.
- Inputs information in SAMIS system and Credible EHR for all services rendered as services are rendered.
- Administers and scores all screenings and maintains scores in SAMIS system and Credible EHR.
- Ensures participant demographic and contact information is accurate.
- Follows –up with families regarding questions within at least 2 days and with referrals within at least 7 days with assistance of Developmental Specialist.
- Provides referrals to all families as scheduled and as needed based on screening scores and concerns of pediatrician.
- Assists with follow-up of client compliance.
- Demonstrates flexibility and change oriented.
- Complies with CHI's Policies and Procedures at all times.
- Adheres to Confidentiality Policies and Procedures / HIPAA Regulations.
- Completes mandatory in-service trainings.
- Communicates and solves problems through the proper chain of command.
- Provides services in a sensitivity non-judgmental and non-discriminatory manner to a diverse population.
- Attends required staff meetings.
- Maintains flexibility in regard to expected or unexpected changes in the work environment.
- Maintains compliance with regulatory entities and grant requirements.
- Completes B&E Forms within the same day of intervention if necessary.
- Performs other duties as assigned.
