Physical Therapist (PT) – Skilled Nursing Facility
Role OverviewProvide physical therapy services in a Skilled Nursing Facility setting, evaluating residents, developing personalized treatment plans, and collaborating with care teams to improve patient mobility and outcomes.
Key Responsibilities- Assess residents and create individualized treatment programs
- Deliver therapy to both rehabilitative and long-term care patients
- Accurately document treatments, progress, and outcomes
- Work with interdisciplinary teams to optimize patient care
- Active California Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Preferred experience in SNF or geriatrics
- Strong communication and documentation skills
- Ability to start immediately
Multiple positions available with potential for permanent placement.
Compensation ranges from $60–$65/hour, with weekly pay.
The role involves a day shift, 36 hours weekly, with flexible start dates.
