Community Health Navigator (Part-Time)

Overtown Youth Center is seeking a dedicated and community-focused Community Health Navigator (CHN) to join our team in a full-time capacity. This essential role will serve as a trusted liaison between OYC Miami and the families we support, delivering culturally relevant health education and assisting community members in navigating both social and health-related resources. The CHN will play a key part in addressing barriers to care while advancing OYC Miami's mission to inspire, empower, and enrich youth and families across our Health, Family, and Economic Sustainability pillars.

In addition, the CHN will support strong community engagement efforts, facilitate resource linkage, and leverage OYC Miami's network to connect families with health services, educational opportunities, financial assistance, legal aid, and employment supports. This position is ideal for someone committed to fostering meaningful relationships, promoting family engagement, and ensuring that children and families – particularly those in Overtown and surrounding neighborhoods – receive comprehensive, wrap-around support.

What you would do:
  • Build strong, trusting relationships with youth, families, and community partners through outreach, home visits, community events, and ongoing engagement.
  • Assist families with accessing health, social service, and community resources, including SNAP, Medicaid, housing assistance, and other benefits.
  • Connect participants to local health providers, clinics, and OYC-based services.
  • Provide one-on-one support, health coaching, and informal health education related to chronic disease prevention, nutrition, mental well-being, and other relevant topics.
  • Conduct needs assessments with community residents to identify health needs and gaps in service access.
  • Facilitate health education workshops or small-group sessions that support behavior change.
  • Offer guidance on medical appointment preparation, treatment adherence, and transportation resources.
  • Conduct regular outreach in Overtown and surrounding areas to promote OYC Miami programs, participate in community events, and strengthen partnerships.
  • Serve as liaison between students, families, and community partners to enhance resource access and engagement.
  • Ensure accurate documentation of client interactions, referrals, outreach logs, and service notes.
  • Maintain confidentiality and adhere to HIPAA regulations at all times.
  • Comply with grant requirements, timely reporting, and internal quality assurance processes.
  • Assist clients with navigating and using the OnMed telehealth kiosk, ensuring a positive and supportive experience.
  • Maintain up-to-date client information within OYC Miami record systems.
  • Complete the Community Health Navigator Training Course and all required certifications.
Requirements Who we’re looking for:
  • High school diploma or GED required
  • Willingness complete Community Health Navigator training program within 6-8 months.
  • Minimum 1 year of experience working in Overtown community or similar urban neighborhoods or with youth/family-focused services preferred.
  • Bilingual preferred (Spanish or Haitian Creole).
  • Strong interpersonal, organizational, and communication skills.
  • Demonstrated ability to build trust with individuals from diverse backgrounds.
  • Valid driver's license, reliable transportation, and the ability to pass drug screening and Level 2 background check.
