East Ridge Village is seeking a full-time Home Health Coordinator!

JOB SUMMARY:

The Home Health Coordinator is responsible for ensuring that Home Health personnel provide in-home care and support to clients in their homes. In addition, this individual provides training and coaching, as necessary, under the direction of and in collaboration with the Home Health Manager and Registered Nurse, to Home Health Aides, Aides, and CNAs. This position reports to the Home Health Manager.

ESSENTIAL

JOB DUTIES:

  • Performs evaluations alongside the nurse and completes the necessary paperwork to sign prospective clients up for appropriate services and requests support from the Home Health Manager and Administrator as needed.
  • Personally visits current associates and residents in the field to ensure adequate work performance.
  • Communicates observations or reported concerns regarding resident needs to the ERRV Home Health team.
  • Provides ongoing, timely performance feedback and coaching to assigned direct care associates.
  • Conducts annual and as needed performance evaluations for assigned associates.
  • Serves as the liaison between the client, direct care associates and the Scheduling department and/or schedules appointments and shifts.
  • Communicates with healthcare providers, families and responsible parties of residents as necessary to coordinate care.
  • Maintains and ensures compliance with ERRV policies and procedures and AHCA regulatory standards.
  • Personally visits clients per regulatory policy timing guidelines to ensure quality care.
  • Maintains accurate, detailed reports and records in compliance with regulatory standards.
  • Attends required meetings.
  • Participates in the on-call rotation and schedule as assigned.
  • Discuss Advance Directives with Resident and/or Resident Representative and ensure updated documentation is on file and shared with appropriate team members.
  • Assist with and/or complete associates' payroll and schedule.
  • Assist with and/or complete Residents long term care insurance, accurate billing.
  • Corresponds with Associates, Residents and/or Resident Representatives.
  • Promotes and participates in the education of staff.
  • Performs other duties as assigned.
