Responsibilities
- Provide direct patient care to residents in our skilled nursing facility, including administering medications, wound care, and conducting vital sign checks
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report changes in resident condition to the healthcare team and physicians
- Maintain accurate and up-to-date records of patient care and treatment
- Collaborate with therapy staff to ensure successful rehabilitation outcomes
- Participate in quality improvement initiatives to continuously improve patient care and satisfaction
