Faith Healthcare Services is seeking compassionate and dedicated Physical Therapists (PTs) and Occupational Therapists (OTs) to join our growing home health team. If you're passionate about helping patients regain independence and improve their quality of life.

We have immediate opportunities for therapists to provide one-on-one skilled therapy services to patients in their homes throughout Miami-Dade County, including Miami, Hialeah, Miami Gardens, Kendall, Homestead, Doral, Cutler Bay, North Miami, Miami Beach, Coral Gables, and surrounding communities.

• Perform comprehensive patient evaluations
• Develop individualized treatment plans
• Provide skilled physical or occupational therapy services in the home
• Educate patients and caregivers on treatment plans and home exercise programs
• Document patient progress and treatment outcomes accurately
• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure quality patient care

• Active Florida Physical Therapist (PT) or Occupational Therapist (OT) license
• Home health experience preferred
• Strong clinical assessment and documentation skills
• Excellent communication and patient education abilities
• Reliable transportation
