Psychiatric Nurse Practitioner

Job Summary

The Psychiatric Advanced Practice Provider, in collaboration and consultation with consulting physicians and other health care professionals, provides holistic, compassionate, medically necessary behavioral health treatment services to patients/residents and provides care coordination and care management activities.

Essential Job Functions

* Delivers cost-effective, quality care to assigned residents/participants

* Provides psychiatric consultation and management of patients across the organization, as assigned. This is likely to include, but not limited to, PACE participants, MJH nursing home residents, DGH hospital patients, research participants, and community-based patients.

* Works with CMO of organization and CMO of behavioral health, in conjunction with other team members, to develop Behavioral health programs and initiatives organization wide.

* Utilizes metrics and behavioral health tools to optimize population health initiatives and to optimize individualized health care plans.

* Complies with the documentation requirements and protocols of organization and contracted payors.

* Manages behavioral chronic and acute conditions effectively in collaboration with the attending physician and works collaboratively with the primary care team.

* Leads the behavioral health team in clinical matters- works closely with behavioral health

* nurses, psychologists, etc…

* Participates in Interdisciplinary Care Team meetings as needed or required by payors or as requested by MJH/

* Follows the course and status of resident while in hospital or SNF and updates the MJH care team.

* Follows approved practice protocols, guidelines, critical pathways, policies and procedures.

* Works collaboratively with physicians, case managers, social workers, family members, key care givers and any appropriate ancillary medical personnel as appropriate.

* Identifies overall quality improvement activities through assessing and planning health care which is individualized to the resident.

* Promotes and maintains lines of communication between department personnel, Nursing Services, Physicians, patients, and families.

* Actively participates in IDT/family/patient conferences, as well as conferences as needed.

* Develops, and provides educational programs to staff, residents and caregivers as needed within the scope of behavioral health services.

* Participates in departmental activities, meetings and in-services and follows established guidelines

Job Requirements

Education

* Graduate from an accredited school of nursing, as an R.N.

* Advanced training and certification as an Advanced Practice Registered Nurse (APRN)

Experience

* Minimum of 5 years of experience as an ARNP or PA

* Experience with value-based care strongly preferred

Licenses/Certifications

* Current APRN license by the Florida State Board of Nursing

* Current Advanced Nurse Practitioner Certification (ANCC, AANP) and applicable Psychiatric Board Certification

* Current ACLS Certification

Abilities Required

* Excellent verbal and written communication skills

* Skills in clinical decision-making, appropriate prioritization, and time management skills

* Demonstrates a caring attitude toward MJH residents, patients, family members, employees, and other members of the care team, internal and external

* Able to think and act calmly to meet unusual occurrences of the job

* Normal working condition with adequate lighting and ventilation

* Exposure to toxic or caustic chemicals; bio-hazardous waste; flammable, explosive gas

* Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from 50-150 pounds
