Responsibilities- Medication Management & Optimization:
Collaborates with healthcare providers to adjust medication therapies, interpret lab results, and support clinical decision-making within a protocol-driven framework. - Comprehensive Medication Reviews:
Conducts detailed evaluations of medication regimens addressing safety, efficacy, cost considerations, side effects, and barriers to adherence. - Chronic Disease Management:
Proactively manages conditions such as diabetes, hypertension, anticoagulation, and asthma to improve control and prevent complications. - Care Coordination & Transitions:
Facilitates seamless transitions of care by resolving medication discrepancies during hospital discharges and preventing readmissions. - Patient Education & Engagement:
Delivers personalized counseling on medication use, side effects, and adherence strategies to empower patients and improve compliance. - Lab Testing & Monitoring:
Orders, interprets, and utilizes laboratory results to monitor medication safety and effectiveness. - Collaborative Practice & Protocols:
Works directly with physicians and providers to implement and follow care protocols, including independent medication adjustments when appropriate. - Administrative & Support
Activities:
Assists with physician consultations, provides accurate drug information, and supports the department’s quality improvement initiatives. - Performs other duties as assigned and modified at manager’s discretion.
Skills and Abilities
- Knowledge of and experience with state and federal insurance programs
- Familiarity with appropriate local, state, and federal pharmacy-related regulations
- Ability to work effectively both independently and as part of a multidisciplinary team
- Excellent organizational, communication, and clinical decision-making skills
- Strong attention to detail and ability to manage multiple priorities
- Willingness to travel locally up to 50% of the time; flexible to work evening, weekends, and/or holidays as needed
- Fluency in spoken and written English
- This job requires use and exercise of independent judgment
- Doctorate of Pharmacy (Pharm
D) required - Valid pharmacy license in good standing (Pharm
D) required - Pharmacy residency or ambulatory care certification (e.g., BCACP) preferred
- Minimum two (2) years of relevant clinical experience in primary care, ambulatory care, managed care, or related setting
- Certification in immunization and medication therapy management is a plus
- Residency training or ambulatory care certification is highly preferred
- Experience in managed care, HMO, PBM, or MSO settings is a plus
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross required within first 90 days of employment
$118,062 - $168,658. The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
