COMPENSATION: The compensation range for this position is $55,000–$65,000, with the opportunity to earn additional income through a variable compensation (commission-based) structure, offering higher earning potential based on performance.

Position SummaryVitae Health is seeking a dedicated weekend Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility every weekend. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.
Key Responsibilities- Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing setting
- Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizations
- Deliver timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well-being
- Supportvalue-based care initiativesby focusing on quality outcomes, reducing avoidable hospitalreadmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvement
- Collaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement efforts
- Facilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and discharges
- Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long-term health
- Maintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standards
- Master’s degree in Nursing from an accredited Nurse Practitioner program
- Active and unrestricted Advanced Practice Nurse (APN) license in respective state
- Current DEA registration and prescriptive authority (or eligibility)
- Board certification as a Nurse Practitioner (e.g., FNP, AGNP)
- 2 years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settings
- Strong clinical assessment, diagnostic, and treatment planning skills
- Knowledge of chronic disease management and geriatric care principles
- Ability to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizations
- Proficiency in evidence-based practice and clinical decision-making
- Ability to work both independently and collaboratively within an interdisciplinary team
- Excellent communication and interpersonal skills with residents, families, and staff
- Strong organizational, time management, and documentation skills
- Familiarity with electronic health records (EHR) systems
