Resident Services / Vitality Nurse

Director of Clinical Operations

JOB SUMMARY

This individual is responsible for attending to the care needs of East Ridge residents while integrating home health nursing services within the community setting. This position maintains and enhances a therapeutic environment that promotes independence and quality of life for the independent living residents. The role combines community-based wellness oversight with direct nursing care delivery consistent with home health practice standards.

ESSENTIAL

JOB FUNCTIONS Wellness & Vitality Assessment
  • Schedule and perform Vitality Assessments (wellness assessments) on Independent Living residents, including counseling and educating residents regarding their Vitality Score and referring to appropriate services as needed
  • Conduct Vitality Assessments of potential new residents
  • Remain current regarding the healthcare and cognitive capabilities of Independent Living Residents
  • Facilitate educational sessions for residents on general healthcare topics and chronic disease management, including nutrition, medication adherence, fall prevention, and aging in place
Home Health Services
  • Provide direct nursing care to residents in accordance with physician orders, including medication management, Vital Signs, and other treatments within the nursing of non-skilled Home Health practice.
  • Conduct comprehensive head-to-toe assessments and monitor changes in residents' health status, communicating findings promptly to physicians and appropriate care team members in the Independent Living.
  • Develop, implement, and evaluate individualized plans of care in collaboration with residents, families, and the interdisciplinary team
  • Administer treatments within the nursing scope of practice and complete appropriate charge slips for all services provided
  • Perform medication reconciliation and educate residents and families on medication regimens, side effects, and safe storage
  • Monitor and manage chronic conditions (e.g., diabetes, heart failure, COPD, hypertension) through regular assessment, education, and coordination with primary care providers
