Home Health Registered Nurse; RN

Registered Nurse, Home Health, Part Time Flexible

• Perform in-home nursing assessments and create individualized care plans
• Conduct all certifications & recertifications using OASIS document system
• Provide skilled nursing care such as wound care, medication management, injections, catheter care, and chronic disease monitoring
• Educate clients and families on care plans, medication regimens, and health maintenance
• Supervise and support Home Health Aides to ensure care plan adherence and excellent service
• Maintain thorough and timely clinical documentation
• Communicate with physicians, case managers, and office staff regarding client needs and changes in condition
• Participate in case conferences and quality improvement activities as needed

• Active Florida RN license (required)
• Minimum 1 year of home health nursing experience
• Valid driver’s license and reliable transportation
• Strong clinical skills and sound judgment
• Excellent communication and interpersonal skills
• Ability to work independently in the field while remaining connected with the care team
