Case Manager RN – Full Time
Ensures appropriate utilization of hospital services as well as developing a discharge plan and all appropriate services in a timely manner. Recognizes signs and symptoms of stroke and calls a stroke alert overhead. Verbalizes stroke alert process. Works with stroke patient and family for discharge planning and treatment based on needs, condition and prognosis. Maintains currency in stroke knowledge by completing required stroke training according to deadlines.

Education Requirements:
- Graduate from an accredited RN program.
Experience: Acute care hospital experience preferred.
Skills: Computer, Excel, Word, PowerPoint.
License / Certification:
- Current state of Florida nursing license.
Travel Requirements: May be asked to provide coverage in other UHS facilities.
