Transitions of Care RN 100% Virtual, Care Bridge - Bilingual Preferred Location
Virtual:
This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. Alternate locations may be considered if candidates reside within a commuting distance from an office.
Monday – Sunday with mandatory 2 weekends a month (4, 10 hour shifts). Eastern Time: 8:00AM – 7:00PM. All other time zones: 7:00AM – 6:00PM, 8:00AM – 7:00PM or 9:00AM – 8:00PM.
Responsibilities- Participate in delivery of patient education and disease management interventions and perform health coaching for members across multiple lines for health improvement/management programs for chronic diseases.
- Conduct behavioral or clinical assessments to identify individual member knowledge, skills, and behavioral needs.
- Identify and/or coordinate specific health coaching plan needs to address objectives and goals identified during assessments.
- Interface with provider and other health professionals to coordinate health coaching plans for the member.
- Implement and/or coordinate coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs.
- Use motivational interviewing to facilitate health behavior change.
- Monitor and evaluate effectiveness of interventions and/or health coaching plans and modify as needed.
- Direct members to facilities, community agencies and appropriate provider/network.
- Refer member to catastrophic case management.
- AS in nursing and minimum of 2 years of condition specific clinical or home health/discharge planning experience; or any combination of education and experience that provides an equivalent background.
- Current unrestricted RN license in applicable state(s) required.
- RN compact license is highly preferred.
- BS in nursing preferred.
- Prior case management experience preferred.
- Bilingual in Spanish is highly preferred.
- Experience working with members that have chronic diseases is highly preferred.
- Experience working in home health is preferred.
