About the Role:

The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care within various healthcare settings, including assisted living facilities and clinics across the United States. This position focuses on providing direct patient care, managing wound care, and supporting pain management protocols to enhance patient comfort and recovery. The LPN will collaborate closely with Nurse Practitioners and other healthcare professionals to implement individualized care plans and ensure comprehensive patient education.

Effective communication in English is essential to accurately document patient information, educate patients and families, and coordinate care activities. Ultimately, the LPN contributes to improving patient outcomes by performing clinical tasks such as venipuncture and monitoring patient conditions with professionalism and compassion.

Minimum Qualifications:

• Valid and current Licensed Practical Nurse (LPN) license in the United States.
• Proficiency in reading, writing, and speaking English to ensure clear communication and documentation.
• Demonstrated experience in wound care and pain management techniques.
• Ability to perform venipuncture safely and effectively.
• Experience working in patient care settings such as assisted living facilities or similar healthcare environments.

Preferred Qualifications:

• Previous experience collaborating with Nurse Practitioners or advanced practice providers.
• Additional certifications related to wound care or pain management.
• Familiarity with electronic health record (EHR) systems.
• Training in patient education methodologies and health promotion.
• Experience in diverse healthcare settings to adapt to various patient needs.

Responsibilities:

• Provide direct nursing care to patients, including wound care management and pain assessment.
• Perform venipuncture and other clinical procedures as required to support diagnostic and treatment processes.
• Educate patients and their families on health maintenance, medication administration, and post-care instructions.
• Document patient information accurately and maintain up-to-date nursing records in compliance with regulatory standards.
• Collaborate with Nurse Practitioners and interdisciplinary teams to develop and implement individualized care plans.
• Monitor patient conditions and report any changes or concerns promptly to the healthcare team.
• Ensure compliance with all nursing protocols and facility policies within the assisted living or clinical environment.
