Registered Nurse; RN – Pediatric & Adult Home Health | Private Duty Nursing

• Care for medically complex patients, primarily children, in the comfort of their homes
• Provide personalized nursing care for pediatric and/or adult patients with complex needs
• Coordinate care plans with physicians, therapists, and other team members
• Monitor patient progress and update care plans as needed
• Educate and support families with in‑home care techniques
• Document visits and care notes in the electronic charting system (EMR)
• Collaborate with clinical supervisor and participate in case conferences
• Act as an advocate for patient welfare and coordinate care between patients, families, and other providers
• Develop, deliver, and revise individualized patient‑centered care plans under physician supervision
• Administer medication per agency policy and physician orders
• Maintain medication reconciliation and report changes in patient condition
• Perform initial evaluation visits and reevaluations of patient nursing needs
• Prepare clinical and progress notes for each patient visit
• Assist with preventive and rehabilitative nursing procedures
• Guarantee confidentiality of patient, employee, and agency matters
• Participate in quality assurance and performance improvement activities
• Take on‑call duty nights, weekends, and holidays as assigned

• Active RN license and CPR certification
• Clinical experience preferred; new graduate candidates welcome
• Excellent communication and critical thinking skills
• A love for patient‑centered care, especially in home environments
• Confidence, compassion, and a calm presence during routine care and critical moments
• Strong verbal and written communication skills
• Ability to maintain licensure and completing continuing education as required
• Physical ability to perform the described duties and secure transportation
• Ability to organize work procedures through time management and supervise ancillary disciplines
