The #1 Priority of this position is…

To coordinate the care of our patients and assist with any Medicare issues.

• Review all admission documentation in order to ensure the quality of the services to be provided.
• Responsible for coordination withall field staff, that they follow agency’s policies and procedures.
• Responsible for reviewing the patient’s plan of care and ensuring that it gets properly implemented,including discussing it with the patients as necessary.
• Maintains daily communication withfield staff in order to ensure that all services are provided as per patient’s expectations.
• Responsible for reviewing all physicians’ orders obtained by field staff.
• Verifies the proper delivery ofdaily services as scheduled, including unscheduled emergent visits in assigned community.

Qualifications

• RN License required.
• Home Health experience of knowledge of Clinical Terminology preferred.

• Flexible as to work schedule. Mustbe available to work on weekends, holidays and in emergency situations.
• Must be able to work under pressure and multi-task.

• Demonstrate excellent verbal communications,interpersonal skills and empathetic listening.
