Position Summary

The Mid-Level Practitioner – Floater is a highly flexible clinical resource who provides direct patient care across designated FQHC sites in Southern Miami and the Florida Keys. The practitioner supports continuity of care, patient access, and clinical operations by providing coverage across multiple locations and programs based on organizational needs.

The Floater Practitioner will provide both in-person and telehealth care, including coverage for planned and unplanned provider absences, vacancies, recruitment gaps, fluctuations in patient volume, and other clinical staffing needs.

The successful candidate must be comfortable working across multiple clinical environments and demonstrate a high degree of flexibility, independence, initiative, adaptability, and responsiveness.

Responsibilities
  • Provides direct patient care across designated FQHC sites and clinical programs based on organizational needs.
  • Provides remote patient care via telehealth across assigned FQHC sites as needed.
  • Provides coverage for planned and unplanned provider absences, vacancies, recruitment gaps, and fluctuations in patient volume.
  • Provides comprehensive primary care services, including acute, chronic, preventive, and follow-up care, within the practitioner's scope of practice and clinical privileges.
  • Supports patient access by providing coverage at sites experiencing increased demand, provider shortages, appointment backlogs, or other operational needs.
  • Adapts to multiple clinical environments, workflows, teams, patient populations, and locations.
  • Demonstrates flexibility and responsiveness in accepting assignments and adjusting to changing clinical and operational priorities.
  • Is self-directed and proactive in assisting other clinicians and clinical teams without being prompted, identifying opportunities to support patient care, access, workflow, and team effectiveness.
  • Collaborates effectively with physicians, advanced practice providers, nurses, medical assistants, behavioral health professionals, care managers, pharmacists, and other members of the interdisciplinary care team.
  • Maintains continuity of care through appropriate follow-up, referrals, care coordination, and effective handoffs between providers and locations.
  • Completes timely and accurate clinical documentation, orders, prescriptions, referrals, and results management in the electronic health record.
  • Participates in quality-improvement, patient-safety, population-health, and other organizational initiatives as assigned.
  • Meets applicable HRSA, UDS, Joint Commission, state, federal, and Health Center quality, safety, compliance, documentation, and productivity standards.
  • Maintains required professional licensure, certifications, credentialing, and clinical privileges.
  • Demonstrates professionalism, accountability, teamwork, cultural sensitivity, and commitment to providing high-quality care to the communities served by the Health Center.
Qualifications Required
  • Master's degree in Nursing, Physician Assistant Studies, or other appropriate advanced-practice discipline.
  • Current and unrestricted Florida license as a Nurse Practitioner (NP) or Physician Assistant (PA).
  • Current national certification appropriate to the practitioner's discipline.
  • Current DEA registration, as applicable.
  • Current BLS certification.
  • Successful completion of the Health Center's credentialing and privileging requirements.
