Position: LTSS Service Coordinator - Case Manager (Region D: Hillsborough/Mantee/Polk)

Job Title

LTSS Service Coordinator - Case Manager (Region D: Hillsborough/Manatee/Polk)

Schedule and Location

Monday‑Friday 8am‑5pm EST. This is a primarily field‑based role. Candidates must reside in Hardee, Highlands, Hillsborough, Manatee, or Polk County (or be within a reasonable commuting distance of an employee office).

Field Overview

The role enables associates to operate mainly in the field, traveling to client sites or designated locations, with occasional office attendance for meetings or training.

Responsibilities
  • Conduct face‑to‑face program assessments and utilize tools for evaluating individuals’ health needs.
  • Apply motivational interviewing techniques for evaluations, coordination, and management of individual waivers (such as LTSS/IDD), BH or PH needs.
  • Identify members with potential clinical health care needs, coordinate care with the interdisciplinary team, and serve as the single point of contact.
  • Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective utilization of services.
  • Document short and long‑term service and support goals in collaboration with the member’s care team.
  • Identify members who may benefit from alternative levels of service or other waiver programs.
  • Serve as mentor or preceptor for new staff and participate in process improvement initiatives.
  • Submit utilization/authorization requests and manage documentation to align with individual care plans.
  • Report critical incidents to appropriate internal and external parties (e.g., Adult Protective Services, Law Enforcement).
  • Assist in appeals, grievances, and state audits.
Qualifications

Minimum Requirements: BA/BS degree and a minimum of 2 years experience working with a social work agency (or equivalent education/experience).

Preferred Skills, Capabilities and Experiences:

  • BA/BS degree in a health care related field.
  • Bilingual in English/Spanish strongly preferred.
  • Experience in LTSS, case management, social work, or hospital discharge planning.
